About This Policy

Eric St Physio (“we”, “us”) is a physiotherapy practice at Suite 13, Level 2, 36 Eric Street, Cottesloe WA 6011. We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and by our professional obligations as registered health practitioners.

This policy explains what information we collect, why we collect it, who we share it with and how you can access or correct it.

What We Collect

Your name, date of birth, address, phone number and email address.
Health information: your symptoms, medical history, medications, assessment findings, treatment notes, imaging and correspondence from other practitioners.
Medicare, DVA, NDIS, private health fund or insurer details where these apply to your care.
Payment details, which are processed by our payment provider and not stored by us.
Information you give us through this website, including enquiry forms and newsletter subscriptions.

How We Collect It

Usually directly from you, in person, by phone, by email or through our online booking and enquiry forms. Sometimes we receive information about you from a third party, such as a referring GP, a specialist, a hospital, an insurer or a family member, where you have consented or where the law allows it.

Why We Collect It

To assess, diagnose and treat you, and to plan your ongoing care.
To communicate with you about appointments, programs and results.
To bill you and to process claims with Medicare, insurers and health funds.
To meet our legal, professional and insurance record-keeping obligations.
To send you updates you have asked to receive. You can unsubscribe at any time.

Who We Share It With

We share your information only where it is necessary for your care or where the law requires it. That may include your GP, specialists or other treating practitioners; your health fund, insurer, Medicare, DVA or NDIS; our practice management and secure messaging providers; and our professional indemnity insurer or legal advisers if a claim arises.

We do not sell your information, and we do not disclose it overseas except where a service provider we use stores data outside Australia under contractual privacy obligations.

Storing and Protecting It

Clinical records are held in our practice management system, Cliniko, which stores data securely and under Australian privacy obligations. Access is limited to staff who need it. We keep adult health records for at least seven years from your last visit, and records for children until they turn 25, as required by law.

This Website

If you submit an enquiry form or subscribe to our updates, we collect the details you enter and use them to respond to you or to send the updates you asked for. Form submissions are handled by our website host and forwarded to us by email. We may also use website analytics to understand how the site is used, which collects general information such as pages visited and approximate location, not your identity.

Accessing or Correcting Your Information

You can ask to see the information we hold about you, or to correct it if it is wrong, by contacting us at ericstphysio@o2active.com.au or 08 6385 7808. We will respond within 30 days. In limited circumstances we may refuse access, for example where it would pose a serious risk to someone’s life or health, and if so we will explain why in writing.

Making a Complaint

If you are concerned about how we have handled your information, please contact us first so we can try to put it right. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or on 1300 363 992.
